PROTEUS SPP.

PROTEUS (BACTERIA)

Proteus is a member of the Enterobacteriaceae family. The genus of Proteus consists of motile, aerobic and facultatively anaerobic, Gram-negative rods. Proteus is a member of the tribe Proteeae, which also includes Morganella and Providencia. The genus Proteus currently consists of five named species: P. mirabilis, P. vulgaris, P. penneri, P. myxofaciens and P. hauseri and three unnamed genomospecies: Proteus genomospecies. However, a recent study indicated that P. myxofaciens may represent a separate genus with low similarity to tribe Proteeae, and it has been suggested that this organism be renamed Cosenzaea myxofaciens.

A striking microbiologic characteristic of Proteus species is their swarming activity. Swarming appears macroscopically as concentric rings of growth emanating from a single colony or inoculum. On a cellular level, swarming results from bacterial transformation from "swimmer cells" in broth to "swarmer cells" on a surface such as agar, in a process involving cellular elongation and increased flagellin synthesis. The genus name Proteus originates from the mythological Greek sea god Proteus, who was an attendant to Poseidon. Proteus could change his shape at will. This attribute reminded early microbiologists of the morphologic variability of the Protei on subculture, including their ability to swarm.

Members of the genus Proteus are widespread in the environment and are found in the human gastrointestinal tract. The most common infections caused by Proteus spp. are urinary tract infections (UTIs). Proteus spp. can be found to colonize the vaginal introitus prior to onset of bacteruria. Therefore, like Escherichia coli, Proteus spp. causes urinary tract infections by ascending from the rectum to the periurethra and bladder.

P. mirabilis is by far the most common species identified in clinical specimens. P. mirabilis is a common cause of both community-acquired and catheter-associated UTI, cystitis, pyelonephritis, prostatitis, wound infections, and burn infections, and occasionally causes respiratory tract infections, chronic suppurative otitis media, eye infections (endophthalmitis), meningitis, and meningoencephalitis. It is a common cause of bacteremia following catheter-associated UTI, and in rare cases has been reported to cause cellulitis, endocarditits, mastoiditis, empyema, and osteomyelitis. It has also been suggested that P. mirabilis could have a role in the etiology of rheumatoid arthritis.
P. vulgaris, previously considered biogroup 2, has been reported to cause UTIs, wound infections, burn infections, bloodstream infections, and respiratory tract infections. There has also been one case study of P. vulgaris causing bacteremia and brain abscesses, with the suspected point of entry being the digestive tract.
P. penneri, previously biogroup 1, generally causes UTIs, wound infections, burn infections, bloodstream infections, and respiratory tract infections.There has been one case study ofP. penneri Fournier's gangrene in a child with congenital genitourinary anomalies. There has also been one recent report of P. penneri causing "red body disease" of the Pacific white shrimp Penaeus vannamei. Notably, P. penneri may be incorrectly identified as P. mirabilis due to being indole-negative , and it cannot be clearly resolved from P. vulgaris by 16S sequencing unless using the 16S-23S internal transcribed spacer. Thus, the burden of human infections caused by this organism may be underestimated.>
P. myxofaciens was originally isolated from a gypsy moth and has been isolated from UTIs in India.
P. hauseri, previously considered biogroup 3, has not been associated with infections in humans.



LABORATORY DIAGNOSIS
The members of the genus Proteus are Gram negative, motile facultative anaerobic rods. On culture plates, Proteus species are distinguished by their ability to swarm. Proteus spp. have 2-3mm colorless, flat, colonies on MacConkey agar, whereas they swarm in waves to cover blood agar plates and LB agar plates.

Proteus spp. are identified by the following biochemical characteristics: positive methyl-red reaction, negative Voges-Proskauer reaction, phenylalanine deaminase production, growth on KCN and urease production. P. mirabilis and P. penneri are indole-negative, while other Proteus species are indole-positive. The Proteus genomospecies can be distinguished from other Proteus species based on five biochemical characteristics: esculin hydrolysis, salicin fermentation, L-rhamnose fermentation, and elaboration of DNase and lipase.

SIMPLE STAINING

SIMPLE STAINING 

INTRODUCTION: 

 The coloration of bacteria by applying a single staining solution to a fixed smear is a “simple staining”. It is called as “monochrome staining” because only one dye is used. Basic dyes like methylene blue, crystal violet, safranine, malachite green are used to determine the shape, size and arrangement of bacteria. 



PROCEDURE: 

➤ Prepare the smear and heats fix it. 
 ➤Flood the smear with methylene blue stain and allow it to react for 1 minute. 
 ➤Wash the slide with distilled water, blot and air dry. 
➤ Examine the slide under microscope. 

 RESULT: 
Organisms are stained blue in color.

BASİT BOYAMA

Mikroorganizmaların ve özellikle bakterilerin mikroskobik morfolojilerinin incelenebildiği boyama yöntemidir. Bu amaçla farklı boyalar kullanılsa da en yaygını Löffler'in metilen mavisi çözeltisidir.
Bakteriler, kullanılan boyanın rengini alır.


Basit boyama yönteminde aşağıdaki işlemler sırasıyla uygulanır:

➧ 1.2’de belirtildiği şekilde preparat hazırlanır (yayma, kurutma, tespit). 
➧ Preparatın üzerine boya çözeltisi eklenerek kaplanır. 
➧ Yeterli süre (Löffler'in metilen mavisi kullanılmışsa 30–60 saniye, karbol fuksin kullanılmışsa 5–10 saniye, kristal viyole kullanılmışsa 20–30 saniye) beklenir. Eğer daha uzun süre temas olursa bakteriler ile diğer cisimlerin ayırt edilmesi güçleşir. 
➧ Boya dökülerek preparat distile suyla yıkanır. 
➧ Preparat havada veya kurutma kâğıdı ile kurutulur. 
➧ Mikroskopta incelenir.

UREASE TEST

Objective

To determine the ability of the organism to hydrolyse urea by the action of urease enzyme.


Principle

Urea is a nitrogen-containing compound that is produced during decarboxylation of the amino acid arginine in the urea cycle. Urea is a major organic waste product of protein digestion in most vertebrate and is excreted in urine. Some bacteria have the ability to produce an enzyme urease as part of its metabolism to break down urea. The urease is a hydrolytic enzyme which attacks the carbon and nitrogen bond with the liberation of ammonia and carbon dioxide. It is a useful diagnostic test for identifying bacteria, especially to distinguish members of the genus Proteus from Gram-negative pathogens. Proteus vulgaris is an important and fast producer of urease.

Urease test is performed by growing test organism on urea agar slant or urea broth with phenol red as an indicator with pH6.8. During the incubation period, the organism capable of producing urease enzyme hydrolyse urea and produce ammonia that raises the pH level. As the pH increases, the phenol red changes from yellowish orange as the initial colour of medium to deep pink. Failure of development of a pink colouration due to no ammonia production is evidence of an inability of the organism to produce urease enzyme.

Media

Enzymatic digest of gelatin (1 g), dextrose (1 g), NaCl (5 g), KH2PO4 (2 g), urea (20 g), phenol red (0.012 g), per 1000 mL, pH 6.8.

Procedure

  1. Streak the surface of a urea agar slant with a portion of a well-isolated colony or inoculate test organism on urea broth containing phenol red as the indicator.
  2. Incubate the urea agar slant or urea broth at 37°C for 24-48 hours.
  3. Examine the development of pink colour.
  4. In case of unknown result incubate the tube for 7 days to check for slow urease production.

                               
                             

Results

Positive: Deep pink colouration, Light Orange, Magenta

Negative: No Color change, Yellowish orange colouration


Limitations

  1. Some organisms rapidly split urea (Proteus spp, H. pylori), while others react slowly.
  2. Urea Agar should not be used to determine the quantitative rate of urease activity, as organisms vary in their capability and rate of hydrolysis.
  3. Prolonged incubation may cause an alkaline reaction in the medium and give the false positive result.


Quality Control

Positive: Proteus vulgaris (ATCC13315)

Weak positive: Klebsiella pneumoniae (ATCC13883)

Negative: Escherichia coli (ATCC25922)

Bu test, bakterilerin üreyi hidrolize eden üreaz (urease) enzimini belirlemek amacıyla yapılır. Bu bakteriler, üreaz enzim aktiviteleri ile üreyi parçalayarak karbondioksit ve amonyak oluşturur. Oluşan alkali ortamda, besiyerinin pembeye dönmesine neden olur.
Testte Christensen’in üre içeren besiyerleri (üreli buyyon veya agar) kullanılır.

Proteus mirabilis ve Klebsiella üreaz pozitif Escherichia coli negatif bakterilerdir.




Testin yapılış tekniği;

➨ Mikroorganizma kültürlerinden, petri kutusu veya tüpteki Christensen’in üreli agar yüzeyine tekniğine uygun ekim yapılır. 
➨ Etüvde, 37°C’de 1-5 gün inkübe edilir. 
➨ Renk değişmelerine dikkat edilerek her gün kontrol edilir. (Bazı durumlarda renk değişikliği 5-6 saat içinde meydana gelebilir.) 
➨ Kolonilerde veya tüpte pembe rengin meydana gelmesi pozitif reaksiyonu gösterir. 
Negatif durumda renk değişikliği olmaz.
